How on Earth do you get your name in there? The download is a .pdf.

Many ways to do this, I guess – I opened the PDF in Elements, selected an appropriate text size and dumped it in, then saved out as an image. Or, you could screen capture the card template, paste into Word and add a text box.
Our you can convert the pdf to png. The long way round, but you get there in the end.
Drop the PDF into Photoshop- all the layers (except for the input text layer) are there. Simply add a layer for the text with your name (or actually type in a Google searchbox and do a screenshot, crop, copy, paste into original).
